I understand that, depending on how I choose to pay–using insurance, out-of-network benefits, or private pay–I will be asked to sign a Payment Agreement confirming my choice before starting services. If I’m using BCBS PPO, claims will be submitted on my behalf. If I have a different PPO plan, I’ll pay the $195 session fee at each appointment and receive a Superbill to request reimbursement from my insurance. If I choose to pay privately, I’ll also pay $195 at the time of service, and my care will remain separate from insurance, allowing for more privacy and flexibility in how we work together. *
